Photographs: Two recent 2" x 2" full-faced color photographs with a light, plain background and taken less than 6 months ago. The face must be square to the camera with the mouth closed (teeth cannot be shown). Click here to download photo guidelines.

Countersign the Photo: One of the photographs must be countersigned by the person who has also countersigned the application. Endorse the back of the photograph by writing "I certify that this is a true likeness of (insert the name of applicant)" and add your signature and the date on which the likeness was compared. Applicant's relatives and notary public will not be accepted as countersignatory.
3. Proof of legal status in USA: Copy of US permanent resident card, US visa, or US passport.
4. Authorization Letter: Please click here and print THREE (3) copies of the authorization letter required for TDS to submit and collect your passsort on your behalf. The three letters must each be signed in BLUE INK
5. Order Form: Cover Page and Credit Card Authorization forms

6. Travel Itinerary: Flight itinerary is required to demonstrate urgency of departure for two week processing.
7. Change of Name? If you have changed your name since the issuance of the last passport, provide
the original court order, marriage certificate, or divorce decree.
8. Letter of Consent - Applicants aged between 16 and 18 must have written parental consent for the issue of a new passport.

Information
- Your
application will be hand carried to the U.K. Embassy.
There, the Passport Officer will examine the application and
if all documents are in order, assign a date the passport
will be issued (according to your departure date from U.S.). Processing
time varies according to the departure date from the U.S.
If requesting 1 - 3 days service please call a TDS representative 1-(800)874-5100, before shipping, to ensure availability of this service. If you are applying to renew a passport which has been expired for more than ten years or you are applying to replace an old style blue British passport you may be required to provide additional supporting documentation and in some cases be interviewed.
- If your passport has been lost / stolen or is unavailable you must provide the address at which you were living when the passport was first issued, the next of kin details included on your previous passport application form and the name of the person who countersigned the original application. For additional identification purposes please send your US passport or an enlarged clear copy (front and back) of your US Alien Registration Card and driving license
