• 1 Visa Application Form, fully completed and signed
• 1 Passport-type photograph
• Completed Cover Page and Credit Card Authorization forms (print from browser)
• Letter from sending Government Department or Agency, addressed to:
  
  Embassy of the Republic of Tanzania, Visa Section, Washington DC
  
  The letter must explain all of the following:
• Applicant's name,occupation, and title
• Purpose of travel
• Expected dates of the intended visit
• Contact person in Tanzania
