REQUIREMENTS
FOR U.S. PASSPORT RE-ISSUE
Effective September 26th 2005 passport agencies will no longer amend a passport for such reasons as change of name, and change of name by marriage - a new passport book will be issued.
If your passport was issued more than one year ago, follow the instructions for a PASSPORT RENEWAL
If your passport was issued less than one year ago follow the instructions on this page.
OR, if your passport was limited in validity for one year or less AND/OR issued abroad, follow the instructions on this page.
1.
Completed passport re-issue
application form, DS-5504. You should complete and print the application form online version below.

2. Your valid U.S. passport
3. Submit two new passport photographs
4. The correct documentation required for a name change by marriage or court order. All documents submitted for a name change or a data correction must be the official seal of the issuing authority (see page two of the application form).
5.
Completed Cover Page and Credit Card Authorization forms (print using the Print button on your browser)
6. You must print, then complete and send TWO COPIES of an Authorization Letter addressed: "Attn: U.S. Passport Agency" and stating "I authorize
Travel Document Systems to pick up my passport and to discuss
the status of my application with the Passport Services staff." This letter must be signed by the applicant in BLUE
INK.

7. Copy
of itinerary of air tickets showing departure date from the U.S.
or letter from employer stating immediate international travel
is required
Fees:
There is no charge for the re-issue of a passport issued less than one year ago except when expediting is requested (issued in less than six weeks)
Government Expedite Fee: $60.00
TDS Fees to Hand Carry Application:
$75.00 per passport if required in 10+ business days.
Very urgent
applications can be issued between 1 and 3 business days. For more urgent issue please follow our Washington DC or San Francisco instructions.

Include
credit card authorization, a check or money order payable to "Travel
Document Systems, Inc." for TDS Handling Fee, Consular Fee,
and return FedEx.

document verifying the name change to be made. (i.e. marriage
certificate, court order etc.)

Information
- Your
application will be hand carried to the Passport Agency. There, the Passport Agent will examine the
application and if all documents are in order, assign a date
the passport will be issued, according to departure date from
the U.S. Passport applications submitted by mail to passport
agencies routinely take 6 to 8 weeks to be processed.
- Urgent
issue applications can be issued in between 1 and 3 business days. For urgent issue please follow our Washington DC or San Francisco instructions.

applications by hand carrying them to the U.S. Passport Agency,
and following up on the progress. Please note that TDS is
not in a position to be able to offer any guarantees of passports
to be issued by a specific date. Issue is determined by urgency
of your departure from the U.S.
