Lost or Stolen Passport

 


REQUIREMENTS TO EXPEDITE REPLACEMENT OF LOST OR STOLEN U.S. PASSPORTS


Instructions and Document Completion:

1. Completed application form DS-11 and form DS-64.
  • Applicants requesting expedited passport service through TDS, MUST now use an online version of the form DS-11, rather than handwriting the required personal data.

  • DO NOT SIGN the DS-11 form until asked to do so by the Post Office or Court House passport clerk.

  • PASSPORT CARDS: The new Passport Cards cannot be expedited at this time and take approximately 4-5 weeks to issue. If you are applying for a regular passport only, please mark the box on your passport application next to "Passport Book". If you also want to apply for a Passport Card you should also mark the box next "Passport Card". There is an additional $20.00 government fee for the Passport Card and the Passport Card will be processed separately from your regular passport (in about 4-5 weeks) then mailed to you directly by First Class mail.

  • If replacing a lost passport form DSP-64 must also be completed.

  • You MUST print and submit TWO copies of your completed application DS-11 and form DS-64. One copy for the sealed envelope and the other copy for inclusion in the FEDEX Envelope, paper-clipped to the outside of your sealed envelope (see #9 below).

Two recent professional passport-type photographs

3. Proof of U.S. Citizenship: a) certified birth certificate, issued by state or county health department or vital statistics bureau (not hospital certificates); b) naturalization certificate; c) certificate of citizenship; d) previous U.S. passport.

4. Completed Cover Page and Credit Card Authorization forms (print from browser)

5. Proof of Identity, with photo: (a) valid driver's license, (b) valid college identification card, (c) current military or government i.d. card, (d) a Certificate of Naturalization or Citizenship, (e) a previous U.S. passport, (f) corporate or employer i.d. card.

6. Appointment:

    If you require an emergency passport issued in 1-3 business days, you must have an appointment to proceed. Please call 1-888-874-5100 for an appointment. TDS must receive your document at least one business day prior to your appointment.
7. Print, then complete and include TWO COPIES of a Authorization Letter "Attn: U.S. Passport Agency" stating: "I, (applicant name and date of birth), hereby authorize Travel Document Systems to pick up my passport, and to discuss the status of my application with the Passport Services staff."

This letter must contain an original sigature in BLUE INK.

One copy of this letter, with original signature, should be placed inside your sealed envelope by the passport clerk and the other should be paper-clipped to the outside of that envelope


8. Proof of departure date (two copies) required (copy of itinerary or air tickets showing applicant is departing the U.S.).

One copy of the itinierary should be placed inside your sealed envelope, by the passport clerk, and the other should be paper-clipped to the outside of that envelope.

9. Take the above requirements to your local acceptance agent for Passport Applications. Pay the clerk the U.S. government fee $160.00 for adults, $145.00 for children under 16 years. (Be prepared to write two checks for the total amount, as the clerk may retain $25.00 execution fee). Please write your name and date of birth on the check. Ask the clerk to execute the documents and place them in a sealed envelope addressed to "U.S. Passport Agency".

Processing Information

  • Your application will be hand carried to the Passport Agency in San Francisco. There, the Passport Agent will examine the application and if all documents are in order, assign a date the passport will be issued, according to departure date from the U.S.,
  • Passport applications submitted by mail to passport agencies routinely take 6 to 8 weeks to be processed. TDS expedites applications by hand carrying them to the U.S. Passport Agency, and following up on the progress.
  • Urgent issue applications (which may be issued in 24 hours or less), require proof of departure within 7 business days of being submitted to the passport agency and MUST be shipped to TDS using only the FEDEX Company.
  • Urgent applications can be issued as fast as 24 hours, less urgent applications are issued within seven days.
  • A 48-page business travelers passport can be issued in lieu of the usual 24-page passport on request at no additional charge (however, ONLY if the larger books are available at the time of issue). Attach a note requesting a 48-page passport to the passport application.
