VISA APPLICATION FORMS
This
Visa Application is on two pages of white paper measuring 8 1/2
by 11 (letter size) which must be photocopied back-to-back on
a single sheet. It should not be enlarged or reduced. This application
form should only be used at the Washington embassy. Also available
below is the application form to request an official document
such as Birth, Marriage, or Death Certificate, Educational records,
Civil or Military service, etc.

Document Format (PDF) you will require Adobe Acrobat Reader Version
3.0 or higher, free from Adobe Systems, Inc. You can download
the Acrobat Reader version that is suitable to your system from
the Adobe web site.
