VISA APPLICATION FORMS
This
Visa Application is on three pages of white paper measuring
8 1/2 by 11 (LETTER SIZE), which should be completed online and
then printed. It should not be enlarged or reduced. This application
form should only be used at the Washington embassy.

Format (PDF) you will require Adobe Acrobat Reader Version 3.0
or higher, free from Adobe Systems, Inc. You can download the
Acrobat Reader version that is suitable to your system from the
Adobe web site.
