REQUIREMENTS FOR U.S. CITIZENS TO OBTAIN VISA TO ENTER BRAZIL

TOURIST VISA
Eligibility for Tourist Visa as follows:

Leisure travel (sightseeing only)

Visit to friends and relatives (does not include religious visits)

Scientists, professors, or researchers (who are current University faculty), attending cultural, technological, or scientific conferences, seminars or meeting (services provided must not be paid by organizations/companies in Brazil, except for reimbursement of expenses or per diem allowances. An invitation from the host organization would usually be required.

Participation in artistic or amateur sport competitions, whenever no monetary prize or paid admission is involved. An invitation from the host organization would usually be required.

This information applies to applicants who are residents of the following states in the jurisdiction of the Consulate General of Brazil in Chicago: Illinois, Indiana, Iowa, Michigan, Minnesota, Missouri, Nebraska, North Dakota, South Dakota, and Wisconsin.

Submit U.S. Passport (not a copy), must have be valid for at least 6 months from date of entry

1 Brazil Visa Application Form, fully completed and signed. (The Brazil consulate in Chicago has produced an electronic visa application. Once on the link above, click on "Visa Request" and follow the steps. When completed print a copy of the application and send to TDS along with all other requirements)

Completed and signed Sworn Statement Waiver. Failure to complete this form and enclose it with your application materials will delay the processing of your visa
Proof of Residency - Provide a clear photocopy of your driver license showing residence in the jurisdiction of the Chicago Consulate

Completed Cover Page and Credit Card Authorization forms (print from browser)
Copy of round trip ticket, or itinerary from an air carrier or travel agent attesting to the acquisition of the ticket, showing flight information and dates of departure to and from Brazil.

If the visa application mentions "visiting friends" please provide a NOTARIZED letter of invitation from the host in Brazil. This letter may be faxed provided the notary's seal is legible.
- Your application will be checked and hand carried to the Brazil Consulate. Normal processing time is not less than three weeks.
Minor Children:(Under 18 Years)

U.S. Passport (not a copy), must be valid for 6 months from date of entry, and contain at least one blank visa page.

Notarized photocopy of the birth certificate (not orginal)
In the event of absent parent(s), a notarized letter of parental consent authorizing the Consulate General of Brazil to issue a visa, signed by the absent parent(s) or guardian(s).
Copy of round trip airline tickets -OR- letter from travel agent confirming round-trip tickets will be issued. Itinerary is NOT ACCEPTED unless showing ticket number(s), or that the ticket is paid for.
